Admission Requirements

Additional Information

All applicants must be 17 years of age or older. High school diploma or GED is not required. A criminal background check is required for admission into the program.  An applicant may receive a copy of their background check from the local Sheriff's Department; this must be a recent report within 30 days of registration. Diamond Heart Healthcare Learning Institute may obtain a copy for a fee.  Two forms of identification must be presented at time of registration: State ID card or US Driver's License and Social Security Card. A felony conviction makes you ineligible for our program.  Certain misdemeanors are considered ineligible offenses to enter our program.

During the summer months, rising 12th grade high school students will be considered for admission into the Certified Nursing Assistant (CNA) program with proof of most recent transcript.

If you are under 18 years of age we will need the signature of our parent/guardian on the enrollment agreement. All applicants must be at least 17 years of age. 



Certified Medication Aide

 Must be at least 17 years old. Requires a High School Diploma or GED Certificate.   

Required Items

$180.00 deposit toward tuition or full tuition payment, $25 non-refundable application fee, US valid Driver's License, or GA State ID and Social Security Card.
